Original Course Choices

If you wish to be considered for alternative courses following GCSE results, please list your revised choices below (include all courses even if some were in your original choices).

Course changes are usually possible subject to numbers in the groups and other timetable constraints. The Sixth Form team will discuss these changes with you before any final decision is made
If you did not receive a grade 4 in GCSE Maths or English, please check the appropriate box. It is a legal requirement that you re-sit these subjects.

Do you have any other support needs that the Sixth Form team needs to be aware of?*
Are you a UK passport holder?*

If your family income is below £25,000 you are eligible for the Sixth Form bursary fund. Application forms can be found on the website under Applying to Our Sixth Form/Finance: The 16 - 19 Bursary Fund and should be returned to the Sixth Form team at the start of term.
